General information
Koller MD, PLLC is a small outpatient private practice that offers non-urgent child & adolescent psychiatric evaluation, management and care.
Services include consultations, medication management, psychotherapy and referrals.
We generally only accept patients ranging in age from 6-18. However, exceptions may be made on a case by case determination (for example: adult children with disabilities who are under parental guardianship, older teenagers who are still in high school & living with parents, etc.).
Payment
This is a direct pay clinic. Most major credit cards are accepted.
Unfortunately we do not accept any insurance plans, but we are happy to provide the necessary receipts to our patients so they can file with their own insurance companies for out of network benefits. You can call the number on the back of your insurance card to see how this works.
Hours of availability
Appointment times are available Tuesday-Thursday, 10 am-5 pm.
Routine messages & requests are received/handled Monday -Thursday, 10 AM- 5 PM (no after-hour coverage).
For any after-hours urgent or emergency issues, you will need to call 911 or go to the nearest ER.
Restrictions
Due to limitations of size and staffing, we are unable to accept cases likely to require frequent emergency interventions, specialized treatment settings, affiliate specialists or high administrative loads. However, help in locating appropriate referrals for these type of services may be given when possible. Examples include:
–forensic evaluations
–custody evaluations
–open/pending legal cases
–open CPS or Foster Care placements
–substance abuse disorders
–eating disorders
